I can tell you that the biggest culprit for these broken dreams is a classic mistake: biting off more than you can chew.

I couldn't agree more with this statement. The tension between ambition and practicality is something that big tech doesn't know how to handle well. Especially for data science teams.

Our team handles this really well. I've mostly seen and experienced this in teams with junior DS or in teams with no PM.
