didn't gave this a try, but I've always had this similar idea in my head where a hosted service allows a web-app/website publisher to collect responses from users from within the application easily and quickly.

I launched this because I felt the need for it in may latest projects. Basically now you can measure and manage customer feedback as rigorously as you now measure and manage profits. It's based on the famous Net Promoter Score.
